Job Description
Physical Therapist (PT)
Location: 1200 W Mohave Rd Parker AZ 85344
Contract Type: 13-Week Contract-to-Perm Opportunity
Schedule: Monday Friday 6:30 AM 3:30 PM
Hours Per Week: 40
We are seeking a dedicated and skilled Physical Therapist (PT) to join our team in Parker AZ. This role offers a balanced caseload a supportive team environment and opportunities for both outpatient and inpatient experience within a Critical Access Hospital setting. Ideal for therapists looking to expand their inpatient skills while maintaining a strong outpatient practice.
Key Responsibilities-
Provide comprehensive physical therapy services including daily visits reassessments and evaluations (approximately 30% each).

Deliver patient care primarily in an outpatient setting with some inpatient coverage.

Collaborate with a multidisciplinary team to develop and implement individualized care plans.

Maintain accurate and timely documentation using facility EMR systems.

Achieve productivity goals of 10 patients per day while maintaining high-quality care standards.

Therapy Team Onsite: 2 PTs 4 PTAs 1 OT and 1 Rehab Tech.

Supportive and collaborative work culture with mentorship available for inpatient work.

Exposure to a variety of patient populations in a Critical Access Hospital environment.

Education: Graduate of an accredited Physical Therapy program (Doctorate or Masters in Physical Therapy).

License: Active and unrestricted Physical Therapist license in the state of Arizona.

Experience: Minimum of 1 completed travel contract.
