Position Summary:
Under the general direction of the Coordinator of Support Programs the Community Habilitation Support Professional is responsible for providing services to the individuals consistent with the established Individual Service Plan as well as the In-Home Residential Habilitation Plans.

Assist individuals to achieve and exceed their personal goals.
Demonstrate the ability to support positive behavior.
Demonstrates teamwork with the individual co-workers and families in implementing positive behavioral support strategies consistent with available behavioral support plans.
Support the individuals by providing a comfortable functional and positive environment.
Provide structure and support for the individuals.
Demonstrate respect for all people.
Demonstrates the ability to teach skills to the individuals effectively.
Assist individuals to perform activities of daily living as independently as possible.
Provide training and support for each consumer consistent with the Community Habilitation Plans.
Develop implement and monitor valued outcome goals work with the individuals.
Accurately document valued outcome plans per applicable regulations policy and procedure.
Assist consumers in planning coordinating and attending community inclusion activities within their homes and community.
Assist with all daily living skills as needed.
Participate in ISP and Residential Habilitation case conferences as needed
Accompany consumers on all appointments as necessary.
Maintain on-going communication with the Director or his/her designee.
Attend and actively participate in mandatory training and meetings.
Maintain a flexible work schedule based on the needs of the families served.
Complete paperwork functions as assigned.
Assure consumer safety and protection.
Assist with behavioral issues.
Respect the living space and property of the families served.
Lift transfer and position consumers in wheelchairs as needed.
Able to safely drive the Agencys vehicles.
Attend and actively participate in training programs and staff meetings.
High School Diploma or GED College Degree preferred
Valid and clean New York State Drivers License
AMAP CPR SCIP-R and First Aid certification preferred
Minimum of one year of experience providing direct care support to intellectually challenged individuals is preferred
