The Acero Schools Special Education CaseManager plays a critical role in the academic success of students this position the Case Manager will provide services to eachstudent in her/his least restrictive environment. This may include co-teachingwithin an inclusive classroom setting as well as in small group and individualinstruction within a special education environment. Performance is evaluated on StudentAchievement Planning/Preparation Instruction Classroom Environment andProfessionalism.
The Case Manager functions as an advocate forboth parents and students as a facilitator of student-focused discussions asa resource and coach for teachers and as a problem solver. The case managermust be able to handle high-stress situations with ease exhibit superiororganizational skills and have the ability to problem solve and case manager oversees the flow of paper and personnel demanding aprofessional honest flexible and outcome based approach to education.
Core Responsibilities
Student Achievement and Success
- Increase student achievement via quality instructional methods.
- Design instruction both individual and small group which parallels thegeneral education assessments curriculum.
- Monitor student progress participating in review and revision of IEPas appropriate.
- Ensure that students have appropriately written IEPs.
- Implement plan across all school settings. Plan must be developedfollowing a process that reviews and considers all relevant factors includingassessment data curriculum-based measurements and input from all team members.
- Monitor the planand to ensure program compliance: that required timelines are being observed;that direct services accommodations modifications and other servicesindicated on the plan are being implemented across all school settings; andthat the student is benefitting from the services provided.
Commitmentto School and Classroom Culture:
- Assist other professional staff in identification of learningdisabilities development of planned remediation and evaluation of studentprogress.
- Foster relationships with student families via two annual home visits.
- Possess an understanding of State and Federal Special Education Law andCPS Policies.
- Attend all CPS and Acero Schools Special Education-related meetings.
- Assists with the development of high quality IEPs by monitoring the AceroSchools draft policy at the school level and act as coordinator of IEPimplementation with general education staff.
- Communicate with School Director: must have annual IEP calendar andaccess to student IEPs.
- Notify the School Director if any part of a students plan is not beingimplemented appropriately despite the case managers efforts to correctproblems via consultation with other school personnel.
- Manage Behavior Programs: Assist in the development & implementationof student behavior management programs; communicate with teachers & otherinvolved personnel regarding behavior management programs.
- Participate in building special education team meetings to facilitateon-going and open communication with all team members regarding on-going andupcoming needs related to evaluations and IEPs.
- Maintains cooperative working relationships with parents and throughcontinuous collaboration with staff other school district personnel and othercommunity service agencies; maintains effective and timely written and oralcommunication with parents staff other school personnel and communityagencies.
- Assist in the development & implementation of student behaviormanagement programs; communicate with teachers & other involved personnelregarding behavior management programs.
- Participate in building special education team meetings to facilitateon-going and open communication with all team members regarding on-going and upcomingneeds related to evaluations and IEPs.
- Maintainscooperative working relationships with parents and through continuouscollaboration with staff other school district personnel and other communityservice agencies; maintains effective and timely written and oral communicationwith parents staff other school personnel and community agencies.
Professionalismand Leadership
- Deliver instruction as indicated in IEPs within Least RestrictiveEnvironment guidelines.
- Prepare rigorous individual education plans in consultation with parentsand IEP team members.
- Maintain accurate student data and other records and submit reports asrequired.
- Maintain a 100% compliance rate for completing all Annual ReviewsInitial and Three-Year Reevaluations according to State and Federal law.
- Maintain confidentiality according to State guidelines.
- Confirm related services schedules and goals; ensure that relatedservice therapy is being provided as indicated on each IEP; confer withtherapist(s) if services are not provided as written on IEP; notify Director ifproblem persists.
- Case Managers assumes leadership roles with colleagues and the schoolcommunity; initiates sharing new information learned through trainings andProfessional Development regarding Special Education.
RequiredQualifications
Experience:
- Candidate mustbe able to demonstrate the academic progress of students under his/herleadership and articulate the methods used to achieve those results.
- Knowledge ofdata-driven instructional practices.
- Experience withprogress monitoring related to IEPs and RtI.
- Experience withhigh quality IEPs.
- Co-teachingexperience.
- Ability to workcooperatively and positively with students parents/guardians.
- Experience workingcollaboratively with general education teachers.
- Strong technologyskills.
Education:
Certificates:
- Valid IllinoisSpecial Education License (LBS1) or equivalent.
- Must hold permanent residency in the UnitedStates.
Physical Demands
Nature of workrequires an ability to operate standard business ability to communicate and exchange informationcollect compile and prepare work documents set-up and maintain demands require the ability to stand bend reachhigh and low stoop walk around the classroom and kneel upwards of 60% of addition walking up and down stairs several times be able to lift 15 to 20 pounds for short periods of timeand push and pull 15 to 20 lbs.
Working Conditions
Majorityof work performed in a general office requiresavailability for extended hours plus non-traditional hours required to performjob requires participation and attendance atorganization sponsored events and meetings.
