Job Description

Job Summary:

This role involves providing comprehensive care coordination and discharge planning for patients in an acute hospital setting. The Care Manager will conduct patient assessments develop care plans coordinate services and ensure appropriate utilization of resources. Experience with case management discharge planning and working with patients with complex medical and social needs is essential. A Masters degree in Social Work and a relevant Washington state license are required.
Location: Edmonds Washington United States
Responsibilities:
- Conduct initial patient evaluations and develop individualized care plans.
- Coordinate services with home care agencies rehabilitation facilities and other healthcare providers.
- Facilitate discharge planning and ensure smooth transitions of care.
- Utilize InterQual and Milliman guidelines for utilization management.
- Manage appeals and denials related to healthcare coverage.
- Conduct concurrent and retrospective reviews of patient stays.
- Assess patient response to treatment and document progress.
- Perform biopsychosocial assessments including geriatric depression scales and Mini-Mental State Examinations (preferred).
- Document care delivery goals and progress according to established standards.
- Participate in care conferences and collaborate with interdisciplinary teams.
- Provide patient and caregiver education.
- Comply with all relevant regulatory requirements (CMS HIPAA etc.).
- Utilize electronic health record systems (Epic Cerner).
- Perform other duties as assigned.
Required Skills & Certifications:
- Masters degree in Social Work (MSW).
- Washington state-issued post-MSW license (e.g. Licensed Social Work Associate Independent Clinical).
- Case management and discharge planning experience.
- Experience with patients with communication problems (hearing/speech) community programs (income/insurance) and Adult Protective Services (APS).
- Knowledge of Advance Directives Americans with Disabilities Act and cultural issues.
- Proficiency in documentation according to plan of care and reflecting understanding of coverage criteria.
- Experience with patients with Dementia and Stroke.
- Basic Life Support (BLS) certification.
- Proficiency in Epic or Cerner EHR.
- Understanding of regulatory requirements (CMS CPT coding Department of Health DRGs HEDIS HIPAA ICD-10 coding NCQA OSHA The Joint Commission).

Preferred Skills & Certifications:
- Utilization management experience.
- Experience with patients with Cancer CHF COPD Diabetes HIV/AIDS Immune Deficiency Neurological Disorders Paralysis and Psychiatric Disorders.
- Experience with Mini-Mental State Examination (MMSE) and Geriatric Depression Scale.
- Experience with discharge summaries and documentation of patient/caregiver teaching.
Special Considerations:
- Must have an active Washington state license. If the license is in process the application date and estimated issue date must be provided.
- Candidates who have worked for Providence-affiliated facilities must have been separated for at least three months to qualify.
- Local candidates will be considered at a travel rate.
- Travel experience is preferred.
- Floating between units is required.
