Job Description
Shawnee Health Family Health Liaison

Shawnee Health is seeking a full-time Family Health Liaison for our Shawnee Health Carbondale and Marion OB/GYN locations. This position reports directly to the Nursing Development Coordinator.
Responsibilities
- Enroll dyads who meet eligibility criteria in the Better Birth Outcomes Program.
- Collaborate with outside referral agencies to enroll qualified dyads
- Collaborate with families to identify and mitigate barriers to accessing desired support and services.
- Assist clients to meet familys health and wellness goals
- Promote education to support reduction in maternal and infant morbidity and mortality
- Complete a comprehensive intake and screening that support BBO objectives
- Initiate referrals that address gaps in care and any social determinate of health needs
- Tracking referrals to ensure timely follow-up
- Data entry into IDHS reporting system
- Coordinate care for participants post-partum and assist in choosing medical home for dyads
- Participate in outreach events that promote services available through the BBO program
- May conduct home visits as needed
- Coordinate care for qualified dyads to the Healthy Families Program
Requirements
- Must have a valid Illinois nursing license to practice as a CMA
- Must have a minimum of 1 year of relevant work experience
- Experience in Maternal and Child Health
- Complete CHW training within 90 days of hire
- Current BLS Card
Starting salary $21.10-$23.08
