About the Role:
Manage complex community patients with chronic health and social conditions providing high-quality social work services and biopsychosocial assessments.
Conduct risk assessments related to capacity self-determination domestic violence and elder abuse facilitating Advance Care Planning and substitute decision-making discussions.
Advocate for patients and carers coordinating referrals to services such as My Aged Care NDIS and NCAT Guardianship.
Collaborate with multidisciplinary teams and external agencies to ensure comprehensive culturally sensitive care and effective discharge planning.
Provide clinical supervision education and placements for social work students while adhering to professional ethics and standards.
Lead quality improvement initiatives and represent the social work department on internal and external committees.
About You:
Qualified Social Worker with eligibility for AASW membership and strong clinical experience in aged care and health social work.
Committed to the Philosophy Mission and Values of Mary Aikenhead Ministries and St Vincents Health Australia with high personal integrity.
Skilled in psychosocial assessment crisis intervention advocacy and complex discharge planning within diverse clinical frameworks.
Experienced in managing complex social issues affecting patients families and carers across aged care rehabilitation disability and residential settings.
Proven leader in multidisciplinary teams with excellent communication negotiation and organisational skills and experience in clinical supervision and education.
