Certified Community Behavioral Health Clinic (CCBHC) helps provide comprehensive coordinated mental health and substance use services to the individual. CCBHC utilizes an integrated person and family centered treatment plan to help ensure the individuals needs are being met. CCBHC offers access to care for any individual seeking help for mental health substance use or community support regardless of diagnosis residence or ability to pay. The goal of CCBHC is to provide a one stop shop for behavioral health services to the individual.

SUMMARY DESCRIPTION OF WORK
Professionals at this level are experienced and able to work with a minimum of supervision. Educational requirements are very specific. Decision making problem solving and highly developed interpersonal skills are critical. This position will work within a team environment. The individual will have the ability to sign up for weekday evening/overnight or weekend coverage according to their availability. Frequency of coverage is extremely flexible.
Compensation: between $19.74 and $23.54 per hour (wage is based on total years of relevant experience)
Work Schedule: 37.5 hours per week (Monday through Friday 8:00 a.m. to 4:30 p.m.)

MINIMUM QUALIFICATIONS (one or more of the following qualifications to be met):
- Bachelors degree in psychology social work nursing education AND a minimum of one year of experience in behavioral or mental health services
- High School Diploma and two years of experience within the human services field.
Essential Duties and Responsibilities
- Provides crisis intervention and risk assessment of clients for referral sources such as community members law enforcement or school systems.
- Makes recommendations and referrals to the appropriate levels of care following a clinical consultation. Assessments are provided on-site and face to face.
- Conduct a crisis focused assessment and documentation to be completed within 24 hours of crisis call.
- Establishes an intervention plan with client and referral source l participation which specifies immediate treatment needs and follow-up.
- Follow up with crisis clients 24 hours post crisis and maintain an ongoing case load for 30 days to follow.
- Maintains relationships with key community resources as well as provide referrals to clients.
- Identify needs and goals for clients who are post crisis and serve as an ongoing intervention support.
- Document all case management correspondence.
- Ability to handle complex cases.
- Ability to be on-call during the workday. On-call requires answering and responding to crisis calls during the daytime hours and ability to respond to situations on-site during those hours within the time outlined in agency policy.
- Is dependable and punctual regarding scheduling and attendance.
- Refers to other Heartland Family Service programs and other community services as necessary either concurrent with services or for aftercare.
- Conducts outreach activities off-site with community partners.
- Identifies clients in the community in need of services.
- Refers enrolls and assesses clients in the community setting.
- Maintains clinical records meeting established standards of timeliness and completeness.
- Is prompt and accurate in completing all required reports and other requirements.
- Displays a courteous and caring attitude at all times to the clientele volunteers and visitors of the Agency.
- Cooperates and collaborates with program area staff volunteers and other Agency staff.
- Abides by all specific program and Agency procedures policies and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- *Creates maintains and shares an appropriate and dynamic self-care plan.
- *Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friend-raising efforts.
- *Essential functions of this job are to be performed remotely from home on the HFS remote worksite.
- Performs other program related duties as assigned.
