Job Description

Northwest Human Services is a non-profit Federally Qualified Health Center (FQHC) and has been a leader in providing advocacy quality healthcare and social services in Marion and Polk counties since 1970. We are a mission focused organization providing compassionate and professional medical dental psychiatry mental health and wraparound services for those in our community who need it most - uninsured individuals families the homeless and migrant workers.


Resource Center Coordinator


Location: HOST Program

Job Status: Full-Time 1.0 FTE 40 hours a week

Hours: Monday Friday 8:00 am 4:00 pm



The primary area of focus is the oversight of daily operations at the HOST Resource Center.
You will work on the service floor and in the community to engage with youth to learn more about their current situation and connect them with appropriate resources. You will conduct intakes Coordinated Entry assessments and provide information assistance and referral services in effort to coordinate care and connect youth with appropriate services. The Resource Center Coordinator also works in conjunction with NWHS Case Managers Community Health Workers Outreach team members WSC clinicians and other agency staff in a multidisciplinary fashion to assure that a strong focus is placed on recovery and safety in all lines of our service delivery.

In the case of homeless victims of crimes between the ages of 18-24 the Resource Center Coordinator will assess each individual victims current situation and connect them with appropriate resources including information about and assistance with the VOCA program and victims compensation program. VOCA services provided include information about the criminal justice process information about victims rights referrals to other victims service programs individual advocacy and assistance with applying for public benefits crisis intervention referrals to counseling services and emergency financial assistance.



SPECIFIC DUTIES:

  • Oversee the daily operations of the HOST Resource Center during its regularly scheduled hours of operations. This includes but is not limited to opening/closing the facility ensuring inside areas are cleaned/sanitized after consumer use outside areas are clean of trash and facility repairs are scheduled on-time.
  • Recruit train supervise and support TLP Peer Associate volunteers in their service delivery and duties at the Resource Center. Establish and maintain regular one-on-one and group meetings with the Peer Associates to support their immediate needs case managed objectives and life goals.
  • Ensure that supplies are monitored inventoried and appropriately stored in accordance with agency and/or regulatory standards. Submit a request for ordering to the Service Coordinator as needed.
  • Organize and oversee youth-centered outreach efforts and events often in conjunction with community partners to identify educate and provide basic services to at-risk youth in Marion/Polk county.
  • Ensure that all documentation needed to record and provide services is completed and filed according to program/grant requirements and by the assigned due dates.



QUALIFICATIONS:

  • Associates degree required in social sciences psychology human development education criminal justice behavioral health or related field. Bachelors degree preferred.
  • At least 1-2 years of program coordination or lead role in service delivery required.
  • A combination of 3 years relevant work education or experience in mental health addictions and/or social services field.
  • Familiar with emotional physical and chemical dependency issues faced by at-risk youth.
  • Competency in a multi-cultural environment.
  • Knowledge of community resources and support systems.
  • Ability to work independently as well as collaborate with a multi-disciplinary team.
  • Ability to utilize critical thinking skills to problem-solve be proactive and act decisively
  • Strong verbal and written communication skills.
  • Computer literate with use of Microsoft programs.
  • Must have a reliable means of getting to and from work. Must comply with agency guidelines and not transport clients in private vehicle.
  • Valid Oregon Drivers License and proof of automobile insurance.
  • Maintain current CPR First Aid and Food Handler Certifications.
