Home Health Nurse Practitioner (NP)

Salary and Benefits
- Base salary of $130000
- 10% annual performance bonus (based on base salary)
- Total earning potential of $143000

Responsibilities
- Deliver comprehensive primary care services in patients homes
- Manage Medicare-aged patients with multiple chronic conditions
- Partner with care team members and Medical Assistants for coordinated patient care
- Assist with patient scheduling and follow-up planning
- Maintain accurate documentation of care delivered
- Participate in care planning and quality improvement initiatives
- Provide care within an assigned geographic area using reliable transportation
Requirements
- Nurse Practitioner degree from an accredited program
- Active unrestricted Nevada NP license
- Minimum 1 year of experience in geriatrics family medicine or internal medicine preferred
- BLS/ACLS certification
- Proficient in chronic condition management within a home care setting
- Able to lift and carry up to 20 lbs. of equipment
- Excellent communication and clinical decision-making skills
- Reliable vehicle and valid drivers license
