Salary and Benefits
- Salary starting at $130000 (based on experience)
- Annual 10% performance bonus tied to clinical outcomes
- Total potential annual income is $143000
- Full malpractice coverage with tail
- Health Insurance: Employer covers 100% of employee-only premium on the
- Basic Plan; Buy-Up Plan available for $73.90/month
- Dental Insurance: $0/month for employee-only coverage with $2000 annual max per person
- Vision Insurance: $3.10/month for employee-only coverage with $180 frame allowance every 24 months
- Health Savings Account: Employer contributes $500/year for individuals or $1000/year for families
- Paid Time Off: 15 days annually begins accruing immediately
- Paid Holidays: 10 recognized holidays
- Parental Leave: 6 weeks fully paid leave for all new parents
- Mileage Reimbursement: $0.50 per mile
- Cellphone Allowance: $60 per month

Responsibilities
- Deliver primary care and consultative services in members homes
- Schedule: Monday to Friday 8 am to 5 pm
- Partner with leadership to identify and schedule patients in need of home visits
- Collaborate with Medical Assistants and care teams to ensure quality continuity and patient satisfaction
- Maintain accurate documentation of all services and encounters
- Participate in quality improvement activities and ongoing care planning
- Provide care within an assigned geographic area with reliable transportation
Requirements
- Nurse Practitioner degree from an accredited program
- Active unrestricted Nevada NP license
- Minimum 1 year of experience in geriatrics family medicine or internal medicine preferred
- BLS/ACLS certification
- Proficient in chronic condition management within a home care setting
- Able to lift and carry up to 20 lbs. of equipment
- Excellent communication and clinical decision-making skills
- Reliable vehicle and valid drivers license
