We are seeking a Physical Therapist (PT) to support patients in Medford OR and the surrounding communities. This home health position offers a consistent Monday Friday schedule with no weekends or on-call. The PT will be responsible for evaluating and treating adult patients in their homes across various orthopedic and neurologic conditions. A minimum of 1 year of skilled home health experience is required. The facility aims to reduce windshield time by strategically grouping visits.

• Conduct initial and ongoing in-home physical therapy assessments

• Develop individualized treatment plans in collaboration with physicians and interdisciplinary teams

• Educate patients and caregivers on home exercise programs functional mobility and injury prevention

• Provide therapy services for orthopedic neurologic and post-operative conditions

• Document all assessments treatment plans and progress notes using designated EMR system

• Travel throughout assigned territory to complete daily visits (up to 6 patients/day or 30 productivity points)

• Communicate effectively with care teams to ensure coordinated care delivery

• Comply with all federal state and professional regulations and standards

• Experience:

  • Minimum 1 year of skilled home health physical therapy experience (REQUIRED)

  • Must be able to manage 30 productivity points per week (approx. 5 6 patients/day)

• Certifications Required:

  • BLS (AHA)

  • Oregon State PT License Pending license accepted

• Charting System: Required Any accepted (specific system not mandated)

• First-Time Travelers: Not accepted

• Weekend Requirement: None

• On-Call Requirement: None

• Floating: Not required

• Block Scheduling: Not available

• Guaranteed Hours: Facility may call off 1 shift per 2-week pay period

• Local Radius Rule: Candidates within 50 75 miles are considered local and paid the same rate

• Transportation: Reliable vehicle and valid drivers license required public transportation not acceptable

• Education:

  • Graduate of an accredited Physical Therapy program

  • Doctor of Physical Therapy (DPT) or equivalent entry-level PT degree

• Experience:

  • 1 year in skilled home health physical therapy

  • Strong documentation communication and patient education skills

• License & Certifications:

  • Oregon State PT License Active or pending

  • BLS (AHA) Required

• Orthopedic conditions:

  • Total knee replacement total hip replacement post-op shoulder repair spinal surgery

  • Cervicalgia low back pain spinal conditions

• Neurologic conditions:

  • Stroke rehabilitation Parkinsons multiple sclerosis spinal cord injury TBI

• Functional assessment and mobility training

• Body mechanics instruction

Patient Education:

• Home Exercise Program (HEP) design

• Patient and family education

• Safety/fall prevention

Work Settings:

• Home Health (primary)

• Acute Care SNF Rehab (prior experience valued)
