Job Description
This Registered Nurse position requires providing skilled nursing care case management and potentially hospice care to patients of various ages and health conditions. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. A minimum of two years of experience in a hospital home health or hospice setting is required.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and other healthcare professionals.
- Providing skilled nursing care including case management and potentially hospice care.
- Maintaining accurate patient records and documentation.
- Adhering to all hospital policies and procedures.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
- Block scheduling may be accommodated at the managers discretion.
- Return to office (RTO) is required.
