Job Description
Position: Physical Therapist
Location: San Jose CA 95117
Assignment Length: 06 Weeks
Schedule: (3x12 Hour Shifts 36 hrs/week) Day Shift
Pay Rate: $50 $63/hr
We are seeking a dedicated Physical Therapist to provide exceptional care at our skilled nursing facility in San Jose. As a Physical Therapist you will work in a 130-bed post-acute care environment helping patients recover mobility and improve their quality of life.
The Physical Therapist will conduct thorough evaluations develop individualized treatment plans and deliver hands-on therapy this role the Physical Therapist collaborates with nurses physicians and other rehab team members to ensure continuity of care.
This position offers a stable schedule with AM shifts and includes one required weekend day (Tuesday Saturday or Sunday Thursday). The Physical Therapist will benefit from working with advanced therapy equipment and supportive staff in a compassionate setting.

Responsibilities-
Provide physical therapy assessments and treatments to patients in a skilled nursing facility.

Develop and implement personalized care plans.

Collaborate with the interdisciplinary team to optimize patient outcomes.

Educate patients and families about therapy goals and home exercises.

Document all therapy sessions accurately in the EMR system.

Active California Physical Therapist License.

Current BLS certification.

Skilled Nursing Facility experience preferred.

Familiarity with PointClickCare EMR a plus.
