In this advanced clinical role you will:
- Manage complex medication regimens including vasodilators anticoagulants and advanced heart failure therapies
- Deliver direct patient care under collaborative practice agreements

Position Summary
The Ambulatory Clinical Pharmacist will serve as an integral member of the interdisciplinary care team working collaboratively with providers advanced practice providers nursing staff and other healthcare professionals to optimize medication therapy for patients with chronic conditions. This pharmacist will focus on guideline-directed therapy chronic disease management medication optimization and patient education through a team-based care model.
Essential Duties
- Evaluate initiate titrate and monitor pharmacotherapy for patients in alignment with current clinical guidelines for conditions covered by the patient population covered in the respective ambulatory care clinic
- Practice under a Collaborative Practice Agreement in accordance with CRS& Rule 17 in collaboration with clinical providers to optimize medication regimens through:
- Initiation adjustment and titration of medications
- Identification and resolution of drug interactions therapeutic duplications and inappropriate therapies
- Ordering and interpretation of relevant laboratory tests
- Patient education and counseling
- Provide comprehensive medication therapy management services and chronic disease management to improve outcomes and support safe effective and individualized treatment plans.
- Conduct detailed medication reconciliations and therapy reviews to identify potential drug-related problems and support best practices in prescribing.
- Actively participate in daily huddles and multidisciplinary rounds to support real-time clinical decision-making and care coordination.
- Document all patient interactions interventions recommendations and therapeutic decisions in the electronic medical record (EMR) in a timely and accurate manner.
- Identify and resolve barriers to medication access and adherence. Collaborate with patients providers and financial assistance resources to ensure treatment continuity
- Review complex medication orders to ensure accuracy appropriate destination pharmacy and communication with patients regarding changes.
- Work closely with the Utilization Management and Prior Authorization teams to:
- Facilitate timely prior authorization approvals
- Assist with copay assistance and free drug programs
- Navigate specialty pharmacy processes and 340B opportunities
- Contribute to protocol development clinical research initiatives and implementation of evidence-based practice guidelines.
- Provide medication-related education to providers nursing staff and clinic teams on emerging therapies guideline changes and best practices
- Participate in quality improvement projects and outcomes reporting that align with pharmacy clinic and institutional goals.
- Meet or exceed established Key Performance Indicators (KPIs) and contribute to ongoing program evaluation and enhancement as defined by departmental and institutional goals.
Other Duties
- Be available as a resource for Introductory Pharmacy Practice Experience (IPPE) students and Advanced Pharmacy Practice Experience (APPE) Students. Coordinate with Pharmacy Operations Manager and pharmacy team to facilitate a positive learning environment.
- Performs as a back up to main campus and off-site clinic locations as needed.

Minimum Qualifications
- Education: Doctor of Pharmacy (PharmD) from an ACPE-accredited institution required.
- Work Experience: Postgraduate Year One (PGY1) residency required; PGY2 in ambulatory care or specialized area (cardiology oncology infectious disease etc.) preferred. Five years of recent and related experience in clinical pharmacy practice preferred.
- Special Training Certification or Licensure: Colorado Pharmacist License required. Board certification is preferred. Ideal candidate is either BCPS BCACP or a specialty board certification such as BCCP BCOP BCIDP etc. CPR/BLS required.
Salary Range:$135674.89 - $179430.05
