What you bring to SBHG:
- Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR
- Bachelors degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Drivers License required.
How you will make a difference:
- Provide one-on-one vocational services including resume development job search assistance interview preparation and workplace skills training.
- Conduct employment assessments to identify client strengths preferences and employment goals.
- Establish and maintain relationships with local employers and community organizations to identify job opportunities and advocate for clients.
- Assist clients in navigating job applications onboarding processes and workplace accommodations when needed.
- Provide on-the-job coaching and follow-up support to help clients succeed and retain employment.
- Collaborate with clinical staff case managers and other service providers to integrate vocational goals into treatment planning.
- Document client progress and outcomes following agency and funding requirements.
- Participate in multidisciplinary team meetings trainings and quality improvement efforts. Incorporates the Transition to Independence Process (TIP) into services.
- Documents all services provided clearly and concisely within the Electronic Health Record within the timelines and framework established by Central Star.
