Job Description

As an LPN/LVN who will assess patient health problems and needs takes vital signs administers medication develops and implements nursing care plans and maintains medical records. May advise patients on health maintenance and disease prevention or provide case management.Job Functions:
You will be responsible for leading a team of 4-5 medical assistants providing guidance support and oversight to ensure the delivery of high-quality patient care and efficient clinical operations.
Works closely with the clinic care coach to provide clinical education and guidance to patients
Works closely with primary care doctors to serve patients by providing disease screening administering immunizations
Records patient statistics
Participating in educational opportunities
Obtains health history and/or immunization status by interviewing patient and/or relative
Requirements:
Active LPN license
IV Certified
1-2 years experience as nurse in inpatient or outpatient clinic setting
Multi-tasking and organization skills
Preferred:
Bilingual (English Spanish)
Phlebotomy experience
Experience with Electronic Medical Records specifically eCW (eClinical Works)
Experience with HEDIS
Knowledge of primary care medicine requirements (state specific)

Additional Information
This role is considered patient facing and is part of Humanas Tuberculosis (TB) screening program. If selected for this role you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
