JOB DEFINITION:
The Social Care Navigator (SCN) will identify potential eligible Medicaid members for screening and navigation through the 1115 waiver. The Social Care Navigator will be responsible for following these members and referring them to appropriate services as well as documenting encounters and social care plans.
EDUCATIONAL REQUIREMENTS:
Bachelors Degree or Masters Degree in one of the following fields preferred: Social Work Psychology Education Rehabilitation Occupational Therapy Counseling Community Mental Health Sociology Speech and Hearing Physical or Recreational therapy. Degrees in other related areas may be considered.
EXPERIENCE REQUIRED:
DUTIES AND RESPONSIBILITIES:
1. Conducts outreach to Medicaid members whether referred or located out in the community. This includes participating in community events or tabling.
2. Completes NYS 1115 waiver training as well as attends ongoing meetings.
3. Participates in the development/documentation of encounters and social care plans.
4. Maintains effective communications with clients primary care physicians substance abuse and mental healthcare providers family collateral resources and other Agency staff on behalf of clients.
5. Maintains documents records statistics and other related reports in an organized timely and accurate manner as per policy and procedure.
6. Provides telephonic as well as face-to-face outreach engagement and service planning in the field.
7. Acts as a linkage to community services including medical behavioral residential entitlement and any other needed services per interdisciplinary care plan.
8. Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed.
9. Provides crisis intervention and follow-up on an as-needed basis 24 hours a day 7 days a week.
10. Participate in ongoing supervision training and education as needed to ensure a high quality of service delivery and continued professional growth.
11. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
12. Complete QI Training during onboarding and participate in QI refresher training as needed.
