Oak River Rehab 3300 Franklin St Anderson CA 96007
Schedule: Monday through Friday
Hourly Rate: Starting at $19/HR (DOE)

At Oak River Rehab were seeking a compassionate and organized Social Services Assistant to join our care-driven team. If youre passionate about advocating for residents and supporting their emotional and psychosocial well-being this is your opportunity to thrive in a role that truly matters.
What Youll Do:
Assist with resident admissions discharges and care planning
Support residents and families through transitions and emotional needs
Coordinate with nursing therapy and administrative teams to ensure holistic care
Maintain accurate documentation in accordance with state and federal guidelines
Help plan and implement programs that enhance quality of life
Advocate for residents rights and help resolve concerns compassionately
What Were Looking For:
A heart for service and empathy for residents
Experience in social services or a related field (preferred)
Strong organizational and communication skills
Familiarity with long-term care or skilled nursing environments is a plus
