Position Summary
We are currently seeking a licensed Occupational Therapist (OT) to provide in-home therapy services for adult and geriatric patients in West Houston Katy and Sugar Land. The OT will evaluate patient needs develop customized treatment plans and deliver care aimed at restoring independence in daily activities and promoting safety in the home environment.
This is a flexible PRN or part-time opportunity ideal for OTs looking to build a local caseload earn supplemental income or work with a responsive supportive team.
Key Responsibilities

Conduct in-home OT evaluations to assess patient function cognition and safety

Develop and implement individualized treatment plans targeting daily living activities

Provide therapeutic interventions caregiver education and home safety recommendations

Document patient progress in compliance with agency and payer requirements
-
Collaborate with interdisciplinary team members including PTs nurses and family members
-
Travel to patient homes across the assigned service area
Requirements
-
Active Texas Occupational Therapist (OTR) license
-
Minimum of 1 year of occupational therapy experience (home health preferred)

Current CPR certification
-
Reliable transportation and valid drivers license

Strong communication time management and documentation skills

Familiarity with EMR systems and Medicare documentation standards preferred
