Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing advancing age frailty and serious illness in an acute care hospital setting. The RN will perform patient assessments administer medications develop treatment plans and collaborate with the interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
- Charting in Epic.
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum of two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- BLS certification.
Preferred Skills & Certifications:
- Experience in multiple specialties (Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room).
Special Considerations:
