Job Summary:
This Registered Nurse position focuses on providing skilled nursing care to patients in a medical-surgical setting. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential. Experience in hospital home health or hospice care is required.

Responsibilities:
- Providing skilled nursing care to patients.
- Conducting patient assessments.
- Administering medications.
- Developing and implementing treatment plans.
- Collaborating with the interdisciplinary team (physicians other nurses therapists etc.).
- Performing case management duties.
- Providing hospice care (if applicable).
- Maintaining accurate patient records.
- Conducting patient assessments.

Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum of two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and a personal vehicle for field work (if applicable).
