The Central West Ontario Health Team (CW OHT) is a collaboration of health partners across Brampton North Etobicoke West Woodbridge Malton and Bramalea. As one of the largest OHTs in the province it serves close to 900000 people. Together over 275 partners including home and community care acute care specialty care primary care long-term care mental health and addictions services patients/clients and community support services have come together with the goal of providing better coordinated care for the community.
The Coordinator is accountable for the coordination of the CW OHTs Primary Care Network (PCN). The role provides key administrative outreach and operational tasks and serves as an extension of the team they support with the aim of maximizing the teams performance. This role handles multiple changing priorities facilitates resolution of multiple problems that are administrative in nature and is open to constant challenge and change within a fast-paced addition a core component of this role is outreach and engagement with local primary care providers in the community.
Accountabilities:
- Support outreach and engagement with local primary care providers in the community with the goal of increasing PCN membership
- Manages schedules for the PCN leadership including complex meeting coordination and urgent needs related to PCN affairs
- Develops presentations briefing notes memos meeting agendas and formal/informal minutes for the PCN based on direction of the PCN leadership Manager Integrated Health Systems and/or director/executive lead
- Takes initiative to brief leaders frequently particularly in advance of meetings and to keep advised of changing priorities and issues requiring immediate attention
- Support planning and implementation of PCN events including but not limited to the Annual General Meeting
- Conducts secondary research and links with external partners and experts to obtain information and tools to support project activities
- Responds to confidential complicated and/or routine inquiries from external and internal stakeholders using tact and sensitivity
- Supports the development and ongoing management of a customer relationship management (CRM) tool or equivalent to manage PCN member information
- Liaise with partners to support patient attachment
- Screens time-sensitive or confidential correspondence triages information requests to distribute to OHT partners and prepares routine correspondence
- Prepares documents (e.g. correspondence reports presentations agendas minutes advertisements newsletter articles policies and procedures financial documents etc.) using a variety of software programs (Microsoft Excel Word PowerPoint)
- Establishes an effective electronic filing system and maintains appropriate OHT-related files (hardcopy and electronically on Microsoft Teams SharePoint or equivalent systems)
- Maintains reference materials for the OHT (including internal and external distribution lists)
- Other duties as assigned
Qualifications :
- College degree or diploma (administrative studies business administration office administration or equivalent)
- Experience working with primary care and supporting outreach and engagement
- Minimum of three to five years of office administration experience in a health or human service setting
- Advanced proficiency with office software specifically Microsoft Office the internet and electronic data management systems including but not limited to Microsoft Teams
- Ability to work with a sense of urgency on high prioritized timelines
- Exceptional written and verbal communication attention to detail and proofreading skills
- Superior organizational and time management skills
- Ability to handle sensitive and confidential information in a discreet and professional manner
- Demonstrated ability to work under pressure with tight timelines in a fast-paced startup environment
- Flexibility willingness and ability to work and travel between multiples sites as required including working remotely (e.g. at home using Zoom and Microsoft Teams)

Additional Information :
Hours: Currently days eight hour shifts including weekends and statutory holidays (subject to change in accordance with operational requirements)
Salary:
Minimum: $31.81/hourly
Maximum: $ 39.76/hourly
