JOB DESCRIPTION AND POSITION REQUIREMENTS

Penn State Altoona is seeking applicants for the position of part-time Case Manager to provide support and coordination of care to students.

Reporting to the Co-Director of the Health and Wellness Center an integrated center providing health services and counseling and psychological services (CAPS) the case manager will serve as the primary campus case manager for students with mental health academic and basic needs-related concerns. Additionally this position will provide support to HWC clinicians with referral coordination and follow-up as needed and maintain an up-to-date listing of community resources and referral sources.

This position will serve as the primary point of contact to respond to students in need through intervention appropriate referrals and monitoring and is responsible for timely documentation of all care coordination. This is a collaborative role that will coordinate with campus offices to provide outreach and follow up for identified students of concern to intervene early when academic health and safety unexpected life events and/or mental well-being issues arise. The case manager will also assist students who are facing challenges with adequate food stable housing or meeting other basic needs that may compromise their ability to achieve at their full potential.

This part-time position works in tandem with Penn States academic calendar; working 10 months out of the year having winter break and spring break off along with the months of June and July off.

Hours of work are flexible and generally within the hours of operation of the Health and Wellness Center (Monday Tuesday Friday 8-5; Wednesday and Thursday 8-8)

Job Responsibilities:

Compensation: Hourly wage $22/hour.
