JOB DESCRIPTION AND POSITION REQUIREMENTS
Penn State Altoona is seeking applicants for the position of part-time Case Manager to provide support and coordination of care to students.
Reporting to the Co-Director of the Health and Wellness Center an integrated center providing health services and counseling and psychological services (CAPS) the case manager will serve as the primary campus case manager for students with mental health academic and basic needs-related concerns. Additionally this position will provide support to HWC clinicians with referral coordination and follow-up as needed and maintain an up-to-date listing of community resources and referral sources.
This position will serve as the primary point of contact to respond to students in need through intervention appropriate referrals and monitoring and is responsible for timely documentation of all care coordination. This is a collaborative role that will coordinate with campus offices to provide outreach and follow up for identified students of concern to intervene early when academic health and safety unexpected life events and/or mental well-being issues arise. The case manager will also assist students who are facing challenges with adequate food stable housing or meeting other basic needs that may compromise their ability to achieve at their full potential.
The ideal candidate will:
- Have attained at minimum a bachelors degree in social work or related discipline
- Possess good communication skills
- Have knowledge of and experience in using electronic record keeping systems
- Be able to work independently and collaboratively with minimal supervision across multiple units while adhering to policy including FERPA
- Preference will be given to candidates with professional experience working with young adults within higher education or community mental health or case management settings.
This part-time position works in tandem with Penn States academic calendar; working 10 months out of the year having winter break and spring break off along with the months of June and July off.
Hours of work are flexible and generally within the hours of operation of the Health and Wellness Center (Monday Tuesday Friday 8-5; Wednesday and Thursday 8-8)
Job Responsibilities:
- Coordinate and manage campus Care and Advocacy submissions. This includes facilitating Care Team meetings contacting students and/or delegating outreach to other Care Team members referring students to appropriate on or off campus resources and providing follow-up to both students and those individuals who submit concerns to the Care Report portal. Maintain documentation using the Maxient system. Provide outcome statistics and an evaluative end-of-year summary.
- Coordinate and manage the cases assigned from the office of Student Care and Advocacy at University Park for students who apply for Basic Needs support due to facing hardship such as access to adequate food stable housing or other basics needs. This includes contacting students referring students to the appropriate funding or support resources consulting with Student Aid and Student Care and Advocacy at University Park and following up with students. Case Management activities will be documented using the Maxient system. Evaluate program utilization and outcome statistics annually.
- Participate in scheduled Community of Practice and Student Care and Advocacy meetings.
- Liaison with referral sources regarding obtaining outstanding lab or evaluation results discharge records and other continuity of care needs.
- Maintain up-to-date listing of community resources and referrals sources.
- Participate in HWC and campus activities as needed.
Compensation: Hourly wage $22/hour.
