TheArea Office on Aging of Northwestern Ohio Inc. (AOoA)is seeking a compassionate skilled and organizedWaiver Service Coordinatorto join our dynamic Long-Term Care this role youll partner directly with older adults and individuals with disabilities to coordinate services that promote health independence and quality of liferight in their own homes.
What Youll Do - Conduct in-home biopsychosocial assessments to uncover individual strengths needs risks and goals
- Develop and implement person-centered service plans that evolve with each members journey
- Coordinate Medicaid waiver services payer benefits and essential supports
- Advocate for and communicate with caregivers providers physicians and interdisciplinary team members
- Ensure timely documentation compliance and service excellence for every individual you serve
- Support audits and Medicaid eligibility reviews while upholding confidentiality and regulatory standards
What You Bring - Required:Active Ohio licensure as aSocial Worker (LSW)orRegistered Nurse (RN)
- At least1 year of experiencesupporting older adults individuals with disabilities or managing home/community-based care
- Knowledge of Medicaid waivers case management and community resources
- Empathy resilience and strong organizational skills
- Reliable transportation and a valid Ohio drivers license
