Job Summary:
As a Hospice Scheduler you will play a vital role in coordinating and managing the scheduling of hospice services for our patients. Your primary responsibility will be to ensure the timely and efficient coordination of patient visits and care team assignments. Additionally you will serve as a point of contact for patients families and caregivers to address scheduling concerns and facilitate communication between all parties involved in the care process.
Job Description:
The Work:
- Coordinating and scheduling patient visits including admissions routine visits and specialized services in accordance with patient needs and care plans.
- Assigning appropriate care team members to patient cases based on availability expertise and patient preferences.
- Communicating with patients families caregivers and healthcare providers to coordinate schedules address scheduling conflicts and provide updates on visit schedules.
- Ensuring accurate and complete documentation of scheduling information in electronic medical records and scheduling systems.
- Collaborating with interdisciplinary team members including nurses social workers chaplains and volunteers to ensure coordinated and comprehensive care for patients and families.
- Participating in team meetings case conferences and other collaborative efforts to optimize patient care delivery and outcomes.
Qualifications:
- Prior experience in healthcare scheduling preferably in a hospice or palliative care setting.
- Understanding of hospice care principles practices and regulations.
- Excellent communication and interpersonal skills with the ability to effectively communicate with patients families caregivers and healthcare providers.
- Detail-oriented with strong organizational skills and the ability to prioritize tasks in a fast-paced environment.
- Proficiency in using electronic medical records and scheduling systems.
- Compassionate and empathetic attitude towards patients and their families.
- Ability to work collaboratively as part of a multidisciplinary team to achieve common goals.
