Job Description

(CHS) we believe everyone deserves to live a self-directed fulfilling lifeand were looking for an empathetic and organized Individual Supports Program Coordinator to help make that this role youll provide direct support manage services and supervise team members to ensure that every person we serve receives consistent high-quality and person-centered care.
What Youll Do:
- Support and coordinate services for individuals with developmental disabilities ensuring their goals preferences and needs are reflected in every aspect of their support.
- Develop and implement ISP goals ensuring documentation and follow-through in collaboration with each persons support team.
- Advocate and communicate regularly with families guardians DDS healthcare providers and community partners.
- Respond to emergencies and provide on-call support as needed.
- Ensure compliance with state regulations and CHS policies including safety health medication administration and documentation standards.
- Promote community inclusion by developing natural supports and helping individuals access medical dental and wellness services.
- Participate in quality initiatives team meetings and continuous improvement efforts across programs.

What You Bring:
- Associates Degree in a related field required; Bachelors Degree preferred.
- Minimum of 2 years experience supporting individuals with developmental disabilities.
- At least 1 year of supervisory experience in human services.
- Strong documentation and communication skills.
- Ability to multitask problem-solve and lead with compassion and professionalism.
- Valid drivers license (MA NH RI) and reliable transportation required.
- Must be able to lift 30 lbs and meet the physical demands of the role.
