Join our team at Florida Parishes Human Services Authority as a Social Worker providing Functional Family TherapyChild Welfare (FFT-CW) to youth and families involved in the criminal justice system DCFS or wraparound services. This is a community-based position with services delivered primarily in the homes of families across Tangipahoa St. Helena and Livingston Parishes.

Possession of a current Louisiana license to practice as a Licensed Masters Social Worker (LMSW) Licensed Clinical Social Worker (LCSW) or Certified Social Worker (CSW) plus:
A masters degree in social work.
NOTE:
Permanent status will not be granted in this job title without LMSW or LCSW licensure in Louisiana; Certified Social Worker (CSW) credential will not be accepted for granting permanent status.

Job Duties:
Conduct comprehensive biopsychosocial assessments and develop individualized treatment and discharge plans using the FFT-CW model
Deliver in-home evidence-based therapy to families ensuring continuity of care
Collaborate with the FFT-CW team and receive ongoing support and oversight from FFT LLC
Prepare and submit progress reports to social service agencies and referral sources as required
Assist families in accessing appropriate community resources when needed
Attend required trainings and webinars to enhance skills in assessment diagnosis and treatment of mental health and substance use disorders
Coordinate care with Peer Support Specialists Case Management and Care Coordinators to support family members with complex needs
