At Bay Bluffs- Emmet County Medical Care Facility we pride ourselves on providing exceptional care with compassion and dignity. With a total team approach we combine dedicated administration with an experienced professional staff to offer a variety of services and programs for our residents ranging for short term rehabilitation to long term and dementia focused care. We are looking for compassionate hardworking people to join our team of dedicated caregivers.
Physical Therapists aid in the restoration and enhancement of independent function or maximizing independence and function in those individuals whose living status has been compromised by illness and/or injury. Evaluates and treats patients communicates with families physicians and other Interdisciplinary Team members documents services in the medical record.
Currently this position is Per Diem
JOB DUTIES
1. Provide resident treatment which meets their needs and uses current treatment knowledge in accordance with the Facility or those of contract site clinical procedures. This includes but is not limited to:
a. Performing accurate comprehensive resident evaluation utilizing appropriate evaluation tools within facility-specific time frame
b. Developing effective plan of treatment and obtaining appropriate approvals from referring physician.
c. Communicating with supervisor and other Interdisciplinary Team members regarding resident progress.
d. Instructing residents families and/or nursing staff in specified follow through programs.
e. Interviewing residents and families regarding previous level of functioning life style and current and future expectations.
f. Demonstrating knowledge of appropriate resources for problem-solving and clinical protocol.
g. Ensures MDS documentation is completed according to the MDS schedule.
2. Provide accurate complete and clear documentation in accordance with the Facilitys policies and procedures regulatory licensing payer and accrediting requirements.
QUALIFICATIONS
An employee in this class should have the equivalent of the following knowledge training and experience:
Understanding of the principles of physical modalities employed in physical therapy.
Knowledge of use of prosthetic and orthopedicdevices ambulatory aides assistive and restorativedevices and training.
Communication skills to include diplomacy good judgment and other interpersonal skills. Excellent verbal and written communication skills are necessary.
Ability to work with all types and levels of staff residents and families.
Required training and experience includes Bachelors Masters or Doctorate Degree in Physical Therapy from an accredited university. Two to three years experience in a skill nursing facility rehab environment preferred. Current Physical Therapist License for State of Michigan.
