Job Summary:
This role is a community clinic-based case management position for a social worker. The case manager will conduct initial and 180-day assessments for assigned participants coordinate hospital and skilled nursing facility discharges and collaborate with the interdisciplinary team to ensure safe and appropriate discharge plans. The position requires strong communication and collaboration skills.

Responsibilities:
- Conduct initial and 180-day assessments for participants.
- Coordinate hospital discharges collaborating with hospital discharge planners.
- Coordinate skilled nursing facility (SNF) discharges.
- Collaborate with the interdisciplinary team (IDT) for continuity of care.
- Assist in ensuring safe and appropriate discharge plans.
- Communicate recommendations from the Providence PACE IDT.
Required Skills & Certifications:
- Minimum of one year of experience in a relevant field.
- Current BLS certification.
- Master of Social Work (MSW) degree.
- One of the following Washington State licenses: LICSW LSWAIC or LASW.
Preferred Skills & Certifications:
- Experience with hospital discharges.
- Experience with SNF discharges.
- Experience working within an interdisciplinary team.
Special Considerations:
- This is a community clinic-based position.
- First-time travelers are not accepted; experience as a traveler is preferred.
- No weekend or on-call requirements.
- RTO restrictions should be confirmed with the hiring manager.
Scheduling:
- Five 8-hour days per week.
- Specific schedule details to be determined.
