REACH Mission and Values
The REACH Program of Evergreen Treatment Services works with individuals experiencing homelessness
and behavioral health conditions to help them achieve stability and improved quality of life. The REACH
mission is to foster community health and safety through outreach relationship healing interventions
and systems advocacy for people who use drugs. REACH provides outreach-based-care coordination
multidisciplinary clinical services and supports to access and maintain housing. All services are based in
principles of harm reduction that offer respect and dignity to individuals moving through stages of
change in their lives.
REACH incorporates a racial equity lens that includes naming the impact and actively dismantling
systems of oppression rooted in White Supremacy while addressing the root causes perpetuating
historical trauma and immense suffering in individuals lives. We are committed to building a robust
behavioral health response that diverts people away from jail by rebuilding community and providing
services to ensure those presently marginalized arent just surviving but able to thrive.
REACH offers an array of services ranging from survival support provided where folks are living outside
to linkages to essential resources such as housing assistance to resolve legal issues health care
entitlements and easily accessible treatment for substance use disorders and mental health conditions.

Job Summary:
The Integrated Care (IC) domain at REACH focuses on individuals with complex health issues who live unsheltered and use drugs. Grounded in harm reduction principles the work within this domain requires a comprehensive approach that recognizes the interconnectedness of health home community and pleasure in fostering well-being. At its core this approach recognizes that drug use is often complex influenced by a multitude of factors including social economic and health-related issues demanding an approach to care that considers the whole person and is tailored to the individual according to their unique needs and identity.
- The Integrated Care Intensive Case Management Program provides expansive and individualized case management services to King County community members who are using drugs living unsheltered and experiencing complex medical conditions. Care managers engage clients where theyre at: in the community shelters encampments and housing settings.
- The Integrated Care Supervisor role is part of the REACH team that supports ETS mission through the demonstration of ETS/REACH values in daily work activities acting as a role model for co-workers developing team cohesion and facilitating sound clinical services for clients.
- This role makes a critical difference at ETS by participating in program development and improvement collaborating with coworkers and community partners and supporting staff in delivering quality services.
- The position is full-time onsite and the days and hours are Monday-Friday 8:30AM-5:00PM.
What you contribute to this role Responsibilities:
- Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
- Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
- Oversee high quality intensive case management services:
- Ensure staff provide effective clinical care by engaging their clients in outreach settings developing trusting relationships with clients and service providers using harm reduction and trauma-informed practices and enacting individual service plans aligned with client goals.
- Support staff in connecting clients to appropriate community resources and identifying service gaps.
- Ensure staff document services and handle protected health information properly.
- Supervise Integrated Care Intensive Care Managers:
- Support supervisees professional development and policy/procedure adherence through conducting regular supervision meetings and annual Professional Development Plans.
- Lead hiring and onboarding processes for supervisees providing all the support and guidance necessary to ensure success in the role.
- Maintain quality care provision by ensuring adequate coverage when approving supervisees leave requests and providing direct client care when coverage gaps are unavoidable.
- Facilitate conflict resolution between supervisees and co-workers or community partners.
- Advance Integrated Care Intensive Case Management Program operations:
- Collaborate with other Integrated Care Supervisors to facilitate effective intensive case management services.
- Use data to improve the Intensive Case Management Program ensure equitable client outcomes provide constructive feedback to staff and demonstrate the programs impact.
- Manage the programs client intake and discharge processes.
- Participate as a member of REACH leadership:
- Contribute to ETSs mission and strategic plan.
- Facilitate collaboration among ETS staff and community stakeholders.
- Attend and actively participate in leadership meetings to discuss agency-wide initiatives and concerns.
