Job Description

Position Summary:

The CFC Case Manager (CM) carries out all the duties associate with direct case management on behalf of program clients under contract with the Department of Children and Families (DCF). A Case Manager is expected to carry a caseload of 8-10 children/youth. The CM is responsible for coordinating all aspects of the treatment plan for clients who are placed in these addition this person is responsible for identifying and accessing community resources to support and to assist the children in his/her caseload and for maintaining direct communication with DCF the Agency Leads and supervisory staff as well as the Clinical Supervisor and the CFC Program Director.

Duties and Responsibilities:

  • Intake & Assessment and Service Plan: In collaboration with DCF case workers and collaterals upon being assigned a case Case Manager is responsible for conducting an assessment on the adolescent and establishing a service plan with measurable goals and objectives.
  • Program Management: Responsible for maintaining program quality. This includes but is not limited to participating in staff orientation and training client performance and monitoring and ensuring adherence to service plan. Case manager is also responsible for maintaining accurate documentation of their interaction with client and family members as well as case collateral and key events. This should occur in a timely manner and in alignment with the funding source and CENTROs standards for records. All documentation should be entered into I-Family Net and CENTROs internal electronic health record system.
  • Direct Service: Responsible for providing direct services to client and family. This includes but is not limited to: Skill Building service connection and navigation information and referral and transportation and interpreter services where appropriate to children assigned to the caseload. Case Manager is responsible for carrying a 8-10 clients.
  • Responsible for conducting weekly meetings with client and supervision of treatment foster parents in their home develop and oversee with the assistance/input of treatment foster parents the childs formal treatment plan. The Case Manager is also responsible for providing at least once weekly visit with clients for on-going assessment support and monitoring of progress towards treatment objectives. Additionally the Case Manager maintains contact with birth families and assists with goals of re-unification where appropriate.
  • Coordinate regular meetings with all service providers for each client maintain all client records and program records as required attend staff and group supervision as required and attend appropriate training to enhance skills and stay abreast of relevant legal and regulatory changes.
  • Case Manager should serve as part of our multi-disciplinary treatment team assist in the process of matching of children with appropriate treatment foster families.
  • Special Projects: Expected to participate in Agency-sponsored trainings and special events as well as industry networking and professional development activities such as conferences and workshops as regularly attend and participate in supervision as scheduled.
  • Outreach: Expected to conduct outreach activities among DCF case management staff and Family Networks Network staff so to ensure increased program utilization.
  • Other Duties as Assigned.
  • Mandated Reporter Statute: An individual in this position is considered a Mandated Reporter when on or off duty according Massachusetts Statutes.

Required:

  • Bachelors degree in social work Counseling Education or Related Degree
  • Bilingual in English and Spanish.
  • Minimum three years of experience in Human Services Social Work Education or Case Management
  • Must be a Licensed Social Work Assistant (LSWA) or higher in MA or willingness to obtain a Massachusetts Social Work license (minimum LSWA) within six months of hire.
  • Valid Massachusetts drivers license with a good driving record and access to a reliable vehicle.

Preferred:

  • Familiarity with the foster care system in Massachusetts.
  • Experience with Virtual Gateway and/or Electronic Health Records systems.
  • Previous experience conducting home visits and case documentation.

Physical Demands

  • Able to walk up and down stairs.
  • Able to carry 20lbs bags.
  • Able to drive to and from appointments meetings and conferences.
  • Able to write lengthy reports.
