Open Door is currently seeking a HIV Case Manager / Test Counselor in our Ossining medical center. The the HIV Case Manager / Test Counselor participates in a patient-focused team-based approach to medical care. As part of the medical care team the services provided by the HIV Case Manager / Test Counselor aim to improve patient health outcomes and assist patients in managing their own health care and becoming independent and informed consumers of health care services. Primary responsibilities include providing intensive case management services to People Living with HIV/AIDS (PLWHA) HIV pre- and post-test counseling and HIV prevention education and counseling to individuals and groups.
DUTIES AND RESPONSIBILITIES
CASE MANAGEMENT
- Conducts intake and biannual reassessments to identify psychosocial needs and risk factors care coordination needs and barriers to health care
- Develops implements and updates an individualized Care Plan in partnership with each patient and involved family members/caregivers
- Assesses patients needs related to the social determinants of health (e.g. housing literacy employment food clothing childcare access to medication and health services transportation social support emotional health correctional system involvement refugee or immigration status physical safety and domestic violence)
- Provides concrete case management services to address barriers to care including the social determinants of health
- Serves as a healthcare system navigator to ensure that patients can access crucial aspects of health care both internal and external to Open Door. Ensures that care is well coordinated and facilitates communication amongst medical providers.
- Assesses patients confidence and abilities in managing their own health care
- Assists patients with insurance and prescription assistance program applications paperwork and issues as they arise
- Implements disease specific education and assesses and addresses medication adherence issues under the direction of HIV medical specialists (MDs)
- Serves as liaison between HIV medical specialists and patients
- Case conferences with HIV medical specialist and follows up with patient and/or HIV medical specialist on identified matters
- Documents all patient care activities in the electronic medical record and other required data bases
- Coordinates and facilitates patient support groups as needed
- Establishes and maintains active and positive linkages with other community agencies and participates in inter-agency meetings as assigned by supervisor
- Participates in Quality Improvement meetings and initiatives
HIV COUNSELING AND TESTING
- Carries out HIV counseling and testing activities for those at risk of HIV infection as defined by Open Door policy and within guidelines of NYS Public Health Law
- Delivers positive HIV test results carries out partner notifications responsibilities of the agency and facilitates timely referral to HIV specialty medical care
- Maintains supplies of HIV educational materials and HIV testing materials at assigned Open Door sites
- Documents all patient care activities in the electronic medical record and other required data bases
HIV PREVENTION EDUCATION AND COUNSELING
- Identifies patients at high risk for HIV and conducts risk reduction counseling and education
- Develops coordinates and implements education and outreach programs for internal and external audiences
- Participates in the development of appropriate education and outreach materials
- Develops coordinates and implements staff in-reach activities to ensure awareness of HIV program services increase appropriate referrals for HIV prevention counseling and provide current information on HIV and STI prevention and care
ADDITIONAL DUTIES
- Documents all patient care activities in the electronic medical record clearly and accurately in a timely manner
- Conducts other data tracking and reporting activities as assigned
- Utilizes evidence-based and best-practice techniques on a daily basis in accordance with training (e.g. teach-back method Motivational Interviewing and SMART specific measurable attainable realistic and timely goal setting)
COMMUNICATION
- Informs the Senior Manager HIV Care & Prevention Programs or the Senior Director Community Health of any problems that may affect quality and standards of patient services
- Fosters good communication between Open Door and patients by returning all patient telephone requests in a timely manner
- Demonstrates ability to function in a culturally sensitive manner within Open Doors philosophy and mission
- Serves as a facilitator of planned care within the organization and actively promotes planned care concepts
- Ensures communication among Open Door departments through individual small group or department meetings
PLANNING
- Attends staff and other organizational meetings as designated to ascertain Open Doors short- and long-range plans programs and goals within areas of responsibility
- Collaborates with the Senior Director Community Health and other HIV Program staff to develop and achieve program goals and objectives annually
- Participates in staff development programs educational activities seminars and training opportunities
COMPLIANCE MANAGEMENT
- Maintains patient confidentiality at all times per Open Door policies and procedures
- Adheres to National Patient Safety standards as defined by the Joint Commission on Accreditation of Healthcare Organization and the Open Door Family Medical Centers
- Prepares and maintains documentation in compliance with Joint Commission standards and applicable city state and federal laws
- Meets annual mandatory training requirements
MARKETING
- Participates and represents Open Door at health fairs and community outreach activities
QUALIFICATIONS
- EDUCATION: Bachelor degree in Social Work or equivalent degree in a related field
- Minimum 2 years of experience working with PLWHA or in a case management/care coordination role

- Fluent in English and Spanish
- Culturally competent/sensitive (racial ethnic linguistic LGBTQ physical ability etc.) and committed to inclusive multi-cultural programming

EXPERIENCE
- Experience with working with PLWHA preferred

REQUIREMENTS
- Must have valid New York State Driver license and daily access to car
PHYSICAL REQUIREMENTS
- Manual dexterity and regular hours at computer keyboard
- Approximately 50% sitting and 50% walking/standing
