Job Description

  • Barossa Hills Fleurieu Local Health Network Regional Operations
  • Position location is negotiable including option to work from home
  • Salary:RN/RM1 $74831 - $102103 p.a (pro rata) superannuation leave loading salary sacrifice benefits
  • Temporary part time contract working 56 hours per fortnight up to 6 months

WHAT ARE WE OFFERING

This is an exciting opportunity to join BHFLHN as a Patient Flow Nurse supporting safe timely and effective patient movement from all metropolitan LHN hospitals to all regional public hospitals throughout the state of South Australia. With location being negotiable (with work-from-home flexibility but will require some weekly office commitment) this role offers variety autonomy and the chance to contribute to improving the patient journey and discharge experience. Youll work across a 7-day roster without shift work and be part of a dedicated team focused on delivering coordinated person-centred care.

  • Assist in the coordination of patient transfers and discharges to support safe and efficient patient flow
  • Assess patient needs and collaborate with services and agencies to ensure appropriate care pathways
  • Deliver person-centred nursing care in line with professional standards and BHFLHN policies
  • Contribute to quality improvement initiatives and support the development of staff and students
