Job Description

The Care Coordinator at Griswold Home Care is a key member of the home care team- responsible for ensuring high-quality client care through effective scheduling management communication and compliance support.

This role involves coordinating caregiver-client schedules supporting both client and caregiver compliance and maintaining positive relationships to ensure satisfaction and retention.

Strong organizational skills attention to detail and effective communication with both clients and caregivers are essential requirements for this role.

The Care Coordinator also serves as a key liaison between clients families and agency staff to maintain satisfaction and continuity of care.

Key Responsibilities:

  • Create manage and update caregiver schedules for assigned clients ensuring optimal coverage and continuity of care.
  • Conduct and document client annual assessments ensuring all care plans and files remain current and compliant.
  • Perform 2-week and 6-month client retention visits to evaluate satisfaction and reinforce ongoing engagement.
  • Make regular client customer service calls to address concerns gather feedback and support client retention.
  • Collaborate with caregivers clients and other team members to address scheduling conflicts or care concerns promptly.

Additional Responsibilities:

  • Complete and track caregiver annual reviews and ensure all required documentation and training are up to date.
  • Conduct caregiver reference checks during the hiring and onboarding process to support quality assurance.
  • Provide after-hours/on-call support on a rotating basis or as needed to respond to urgent scheduling or care-related issues.

Qualifications:

  • Strong organizational and communication skills
  • Ability to multitask and prioritize in a fast-paced environment
  • Compassionate and professional demeanor
  • Experience in home care or healthcare coordination preferred

Compensation:

  • Quarterly performance-based bonuses are available based on metrics such as client retention schedule fulfillment and quality of service as well as any other additional assigned roles outside of care coordination.
