DescriptionWe are searching for a Diabetes Educator III- someone who provides assessment goal setting education and behavioral intervention and on-going case management to patients and caregivers to promote diabetes self-management in the inpatient and ambulatory settings. Contributes to the development of position papers guideline development or other literature to advance the specialty. Develops processes to improve transitions of care and ensure appropriate self-management education and resources. Creates innovative strategies to achieve the Quadruple Aim (population health quality of patient care experience costs associated with care and meaningfulness of the providers experience).

Qualifications:
- Associates or bachelors in nursing OR masters in nutrition Public Health Education or Nursing is required.
- CDCES or BC-ADM is required
- 5 years experience diabetes care as a licensed dietician or nurse is required.
Responsibilities:
- Provides individualized diabetes self-management education and training to individuals and groups according to the Scope of Practice Standards of Practice and Standards of Professional performance for Diabetes Educators
- Uses the ADCES 7 Self-Care Behaviors as a framework to teach reinforce and evaluate self-management skills.
- Facilitates coordination and communication with primary care providers diabetes care team and other members of the patients network as appropriate
- Communicates effectively through writing presentations reports and public speaking to include one regional or national presentation or active regional or national committee involvement with defined roles and goals
- Collaborates with inter-professional team and demonstrates ability to affectively provide constructive feedback to team
- Contributes to the development of position papers guideline development or other literature to advance the specialty
- Effectively communicate to provide group and/or individual patient/caregiver education.
