Health Solutionsis a premier wellness center focused on whole person care. With over 400 employees in Southern Colorado you would be joining a mighty team of support and administrative staff clinicians physicians nurses and others in our efforts to improve the health and wellbeing of our community.
Were hiring for a full-time Enrollment Specialistto join our Youth and Family Services office located at 1012 W Abriendo.
What Youll Be Doing
RESPONSIBILITIES: As an Enrollment under the supervision of the Director the Enrollment Specialist will:.
- Provide services to connect clients with resources to be successful in treatment;
- Maintain accurate complete and up-to-date written and electronic records;
- Answer calls and provides support and assistance for clients seeking services;
- Provide exceptional care to all clients;
- Assist clients with scheduling appointments;
- help clients with enrolling in the patient portal and providing instruction on the benefits of this resource;
- Maintain timely caseload management by progressing clients appropriately through treatment and ensuring clients are connected to services requested in a timely and clear manner;
- Participate in team meetings and in case staff meetings and consults with other team members as needed;
- Work collaboratively with community agencies and partners; and
- Perform other job-related responsibilities as assigned.

PHYSICAL REQUIREMENTS:
- Requires standing sitting and walking for prolonged periods
- Occasionally lifts and carries items up to 20 pounds
- Requires working under stress in emergency situations
- Requires frequent use of computer.

Qualifications

- A high school/general equivalency diploma
- Experience working with a multi-disciplinary team
- Comfortable working with individuals with mental illness and substance use
- Computer proficiency required

- Valid Colorado driver license and access to reliable transportation during work hours
- Spanish language skills
