Job Description

WHO WE ARE
ONE KIDS PLACE Childrens Treatment Centre provides rehabilitation and related support services for children and youth (up to the age of 19) and their families living in the Districts of Muskoka Nipissing and Parry Sound. All staff will provide service within an interdisciplinary team environment committed to family-centered practice.

THE OPPORTUNITY

Job type: Permanent Full Time 8:30 AM to 4:30 PM

Effective September 2024 OKP has begun to roll out an Alternate Hours program to provide services to our communities outside of standard business working hours (after 4:30 PM). Staff will be expected to participate which will result in working alternate hours to support this program.


POSITION SUMMARY: Reporting to the Clinical Services Manager the FASD Service Facilitator will deliver child- youth- and family-centred service to children and youth with FASD and/or suspected FASD and their families by leading the development of a strengths-based support plan for children and youth with FASD or suspected FASD. The FASD worker will work with the family service providers and educators to build capacity for supporting the child/youth and identify strategies to meet the specific needs of the child/youth and family based on the best available evidence of FASD supports. The FASD workers will be knowledgeable about FASD and will complement the current efforts of service providers within their community to provide direct intervention support collaboration and system navigation across multiple sectors such as health education youth justice community and social services post-secondary and employment and training. The FASD worker will connect children and youth with FASD their families and service providers to interventions and supports including those developed through the other components of the FASD Strategy.


JOB DUTIES

  • Work with children/youth with FASD or suspected FASD and their families to develop a support plan based on individual strengths and needs and informed by the child/youth and familys vision goals and concerns.
  • Support connections to diagnostic supports (as available) and provide information to families as post-diagnosis support.
  • Support connections to other services and with consent share information and the service plan so the family doesnt have to repeat their story.
  • Work with the family service providers and educators supporting the child/youth to build capacity and identify strategies to meet the childs needs.
  • Explore flexible and innovative approaches for service delivery to meet the needs of children/youth and families and to bring forward innovative solutions.
  • Provide information to parents caregivers and school personnel about characteristics and symptoms of FASD and helpful approaches.
  • If a child/youth has multiple and complex needs and their family requires the more intensive support of a Service Planning Coordinator and the Coordinated Service Planning Process support the connection to a dedicated Service Planning Coordinator and remain involved as part of the Coordinated Service Planning team.
  • Work with youth and their family and service providers across sectors to plan for a transition from child and youth services to adult services if required.
  • At a local system level work to improve awareness of FASD and support local cross-sectoral planning tables (such as Special Needs Strategy Cross-Sectoral Committees) to identify gaps and opportunities for improving supports for FASD.
  • Engage with different cultural groups (e.g. Indigenous francophone to discuss how to best meet needs of children and youth with FASD in their communities.
  • Facilitate parent-to-parent connections.
  • Participate in provincially-offered training to increase their own and their organizations capacity to support children and youth with FASD and their families.
  • Achieves standards/benchmarks and performance indicators set by the Clinical Service Manager.
  • Implements policies/procedures to ensure practices meet or exceed guidelines set by One Kids Place
  • Ensures compliance with service documentation and reporting procedures as outlined by the Clinical Service Manager.
  • Ensures all activities are consistent with the vision mission and values of One Kids Place

QUALIFICATIONS:

  • Bachelors Degree in Social Sciences preferred Social Service Worker Diploma required
  • Experience working with children/youth with FASD and their families
  • Knowledge of child development
  • Strong interpersonal skills
  • Experience working with individuals experiencing trauma
  • Experience with and knowledge of behavioural concept
  • Commitment to strength-based principles of child youth and family-centeredness
  • Experience working with the child and youth service system
  • Demonstrated ability to build and maintain relationships with diverse stakeholders
  • Ability to maintain strong professional boundaries
  • Excellent planning organizational presentation and reporting skills
  • Resourceful and independent with the ability to bring leadership within a team-oriented environment
  • Flexible and able to adjust to changing priorities
  • Excellent communication (oral and written including public speaking) and interpersonal skills
  • Travel valid drivers license and proof of insurance required
  • Bilingual French & English is required
  • Proof of COVID Vaccinations as required by the sector

    COMPENSATION: $26.43 - $32.78 hourly hourly (depending on prior experience as per our Collective Agreement).

Lintervenant en matire de TSAF travaillera avec la famille les prestataires de services et les ducateurs pour renforcer la capacit de soutenir lenfant/le jeune et identifiera des stratgies pour rpondre aux besoins spcifiques de lenfant/du jeune et de la famille sur la base des meilleures preuves disponibles de soutien en matire de TSAF. Les travailleurs spcialiss dans le TSAF connatront le TSAF et complteront les efforts actuels des prestataires de services au sein de leur communaut pour fournir un soutien aux interventions directes une collaboration et une navigation dans le systme dans de multiples secteurs tels que la sant lducation la justice pour les jeunes les services communautaires et sociaux laprs- secondaire et emploi et formation. Le travailleur spcialis dans le TSAF mettra les enfants et les jeunes atteints de le TSAF leurs familles et les prestataires de services en contact avec des interventions et des soutiens y compris ceux dvelopps dans le cadre des autres composantes de la stratgie TSAF.
