POSITION SUMMARY: The position oversees the delivery of integrated CalAIM Community Supports at the Youth Navigation Center (YNC) with a focus on housing stability for transition-age youth. This role supervises Housing Case Managers and Navigators manages client intake and ensures effective care plan implementation. This position supports staff training quality assurance and compliance with documentation standards in the EHR and HMIS. They also collaborate with internal teams external partners and Managed Care Plans to ensure coordinated trauma-informed and youth-centered services.
QUALIFICATIONS:
1 to 2 years of experience in a leadership role in a human services environment with supervisory responsibilities.
Previous Cal AIM experience housing community supports and/or healthcare experience is preferred.
Excellent verbal written and interpersonal communication skills.
Demonstrates knowledge of fiscal and program operation systems
Demonstrated knowledge or ability to learn community resources social service agencies and existing housing inventory.
Demonstrated knowledge of permanent supportive housing initiatives.
Ability to support Housing First philosophy and strategies.
Ability to collaborate with county managed care plans and community service providers.
Pass agency paid criminal justice screening fingerprints.
Pass agency paid TB (Tuberculosis) screening.
Pass agency health screening or drug testing if required.
Valid California Drivers License and proof of valid auto insurance coverage.
Pass agency paid MVR clearance with no more than 2 points if you are under the age of 25 our insurance company will not allow any points.
DUTIES:
Supervise the YNC Integrated Community Supports (CS) team including Housing Case Managers and Housing Navigators; ensure adherence to program goals policies and best practices.
Collaborate with the Housing Manager to identify and provide referrals to local housing opportunities tailored to the needs of transition-age youth.
Provide leadership in the integration of Community Supports (CS) services across YNC ensuring alignment with CalAIM principles and trauma-informed youth-centered approaches.
Oversee program performance in collaboration with the Housing Manager; monitor outcomes analyze data and implement improvements based on findings to enhance service effectiveness.
Manage and refine the new client intake process; coordinate with referral sources to ensure timely and appropriate enrollment into Community Supports services.
Oversee the development and implementation of Care Plans; ensure that plans are person-centered regularly updated and aligned with youth goals CalAIM and grant funding requirements.
Support staff in delivering specialized housing support services such as lease education housing readiness neighbor mediation and landlord engagement to promote long-term housing stability.
Coordinate timing of rental assistance and supportive services with staff and external partners to support youth transitioning to or maintaining permanent housing.
Lead quality assurance and compliance efforts including regular audits of client records case notes and documentation in the Electronic Health Record (EHR) and Homeless Management Information System (HMIS).
Monitor caseload distribution to maintain client safety regulatory compliance and staff support.
Foster strong relationships with community partners stakeholders and referral networks serving as an advocate and ambassador for YNCs Community Supports Program.
Collaborate with the ECM Manager to serve as a liaison with Managed Care Plans ensuring compliance with CalAIM regulations and payer requirements.
Ensure that staff are trained in required documentation practices and use of data systems including the EHR and HMIS; provide ongoing support to maintain compliance and data quality.
Attends agency meetings and contribute to billing process improvements.
Performs other tasks as assigned in support of organizational goals.

Demonstrate thorough understanding of behavioral health service documentation requirements
Navigate complex billing scenarios for diverse service interventions
Strong communication skills to collaborate with clinical and financial teams
Ability to maintain compliance with HIPAA and other healthcare billing regulations
Maintain confidentiality and confidential information in accordance with legal standards and/or agency regulations.
Participate in assigned scheduled agency meetings in-service training conferences and other training as determined by the supervisor. This includes serving as an agency representative at assigned community meetings.
Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.
