Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate with an interdisciplinary team. Strong communication and collaboration skills are essential. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring
- Medication administration
- Treatment planning and implementation
- Collaboration with interdisciplinary team (physicians other nurses therapists etc.)
- Case management
- Hospice care (if applicable based on patient needs)
- Documentation of patient care and progress
- Maintaining patient confidentiality
- Adhering to all relevant policies and procedures

- Valid drivers license car insurance and personal vehicle for field work (if applicable)
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
