Job Summary:
This Registered Nurse (RN) position requires providing skilled nursing care case management and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments administer medications develop treatment plans and collaborate effectively within an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and coordination of care.
- Providing skilled nursing care in various settings (potentially including Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room home health or hospice).
