Job Description
- Treatment Rate: $61 /hour
- Estimated Annual Salary: 92482.00
- Treatment Rate: $50 /hour
- Estimated Annual Salary: 77324.00
- Treatment Rate: $64 $70/hour
- Estimated Annual Salary: 96616.00
- Treatment Rate: $53 $60/hour
- Estimated Annual Salary: 81458.00
- Driving documentation (done at home) meetings scheduling calls etc. $12/hour
- Documentation at point-of-care (completed in the patients home) paid at treatment rate 10 minutes added per visit
- Training/Onboarding: Paid at 50% of treatment rate
- Driving Reimbursement: Paid based on drive time
- Employment Type: W2

- Must be able to lift up to 50 pounds
- Ability to bend stoop and squat as part of treatment activities
- Paid training (2 3 weeks - hybrid format - online in person shadowing)
- Mentorship and clinical support

- Not traditional home health
- Patients are managed under a specialized chronic care model
- Caseloads and zones are productivity-based and designed for efficiency
- Time tracking is mobile app

- Candidates should have reliable transportation and a valid drivers license
