The contractor performs the services set forth below under the general direction and supervision of the Community Services Division. This position provides prevention and intervention services to individuals and families who are homeless or at imminent risk of becoming homeless. The contractor shall serve as the Case Manager and is responsible for assisting homeless families in the Transitional Rapid Re-housing (THRRH) and Permanent Supportive Housing program(s).
Duties include but are not limited to the following:
Reviews customer referral applications; completes assessment form; and completes intake.
Creates a case record for each customer for essential documentation
Assist customers in filling-out program forms; provides information regarding program rules regulations and customer rights
Develops individual service plans establishes goals and reassesses the plan as needed
Sets realistic timelines for established goals and submission of documents in case record
Assesses and refers customers to employment and vocational training programs as needed
Provides customers with updated fair market and subsidized housing lists
Assists customer in applying for mainstream benefits (TCA SSI Food Stamps Medicaid Child Care etc.) as needed.
Provides ongoing monitoring to ensure customers receive the services they need and are eligible
Provides or arranges transportation to customer for housing searches and appointments as needed
Homeless Management Information System (HMIS) - Inputs data on customers and updates all pertinent information and service records as needed
Refers and links customers to appropriate resources weekly or as needed i.e. housing employment training clothing food etc.
Provides follow-up on referrals and contacts the agency or program to ensure that services have been provided.
Contacts/Identifies and documents established and potential resources for program participants weekly
Attends meetings with customers and serves as an advocate and mediator at the customer s request.
Responds to emergencies and responsible for completing incident reports involving customers assigned to their caseload
Reports to designated disaster shelters when activated within 24 hours notice.
