You will be updated with latest job alerts via emailWe are seeking a detail-oriented and organized Open Cases Manager to support our growing homecare team in Buffalo. The ideal candidate will be responsible for managing and following up on all open client cases ensuring timely placements caregiver coordination and overall compliance with agency protocols and state regulations.
Key Responsibilities:
Monitor and manage all active/open homecare cases.
Coordinate with caregivers clients and case coordinators to ensure timely staffing and case updates.
Maintain accurate documentation for each open case in the system.
Follow up with assigned staff to ensure service delivery meets the clients needs.
Communicate with intake scheduling and compliance teams to resolve any delays or issues.
Ensure all necessary paperwork and authorizations are completed for each case.
Assist with audits reports and quality checks related to open case status.
Provide exceptional customer service to clients and caregivers.
Qualifications:
Previous experience in homecare healthcare administration or case management preferred.
Strong organizational and time-management skills.
Excellent communication and interpersonal abilities.
Familiarity with Medicaid MLTCs or other homecare-related documentation is a plus.
Proficient in using scheduling systems and case tracking software.
Bilingual (English/Spanish) is a plus.
