Job Description

Job Summary

The Licensed Mental Health Professional (LMHP) will provide counseling services with the support of a multi-disciplinary team to include primary care and substance use disorder treatment in a Rural Healthcare Clinic. The LMHP will conduct biopsychosocial assessments diagnostics using the DSM-5 individualized treatment plans therapy and counseling for a diverse caseload including adults adolescents and families.

Job-Specific Duties and Responsibilities

  • Performs bio-psycho-social intake assessments diagnoses using DSM-5 criteria develops individualized treatment plans and conducts therapy using evidence-based practices.
  • Collects data from a variety of sources such as patient records interviews observation and team members. Documents patient assessment and progress appropriately; maintains timely complete and accurate records.
  • Works as a member of a multi-disciplinary team. Contributes to team-based care through effective partnerships with patients their caregivers and other providers.
  • Follows up on referrals promptly with treatment recommendations.
  • Meets face-to-face with the patient for immediate crisis intervention behavioral health intervention or other social service-related issues. Participates and monitors the appropriate tracking system for identified patients. Coaches patients/families toward successful self-management.
  • Collaborates with internal and external resources to facilitate a safe transition from one hospital to another facility or home with appropriate services. Identifies barriers and initiates action to resolve.
  • Serves as the point-of-contact advocate and informational resource for assigned patients family care team payers and community resources.
  • Proactively acts as a patient advocate responding with empathy and respect to resolve patient/family concerns.

Required Licenses Certifications and/or Registrations

  • LICSW (Licensed Independent Clinical Social Worker) in the state of Washington or LMFT (Licensed Marriage and Family Therapist) in the state of Washington or LMHC (Licensed Mental Health Counselor) Associate licenses will be considered. Current Basic Life Support (BLS) certification required.
