A Field Nurse is a registered nurse (RN) who provides healthcare services to patients in non-traditional settings such as in their homes schools or other community locations. The RN will assess plan implement and evaluate individualized care plans in collaboration with individuals families and other members of the healthcare team. Reports to: Deputy Chief of Quality Key Responsibilities: - Provide in-home skilled nursing assessment care to individuals in accordance with the physicians plan of care and agency policies.
- Perform comprehensive assessments including physical emotional and social factors.
- Develop implement and evaluate patient care plans and update as necessary based on individuals condition.
- Administer medications and treatments as prescribed and monitor for adverse reactions.
- Educate individuals and families on processes self-care techniques and prevention strategies.
- Coordinate care with interdisciplinary team members including physicians therapists DSPs and case managers.
- Document all individuals care activities accurately and timely in the electronic medical record (EMR).
- Monitor individuals progress and report changes in condition to the physician and care team.
- Ensure compliance with all local state and federal regulations and agency policies.
- Participate in on-call rotation as assigned.
Qualifications: - Current and valid Registered Nurse (RN) license in the state of Indiana.
- Graduate of an accredited nursing program.
- Minimum of one (1) year of nursing experience; home health experience preferred.
- Current CPR certification.
- Valid drivers license and reliable transportation.
- Proficient in electronic charting and documentation.

While performing the duties of this job the employee is regularly exposed to (i) Individuals residences in which the noise levels are usually moderate and (ii) outside weather conditions.
