Salary and Benefits

Responsibilities
- Monday Friday (9am 5pm)
- On-call and occasional death visit (rare)
- Services are delivered in personal homes independent living apartments and assisted living centers throughout Central Ohio
- Total census is approximately 75 patients with 34 patient visits per day
- Utilize KanTime EMR system for documentation and care coordination
- Provide psychosocial support to hospice patients and their families
- Conduct comprehensive assessments and develop individualized care plans
- Assist patients and families with end-of-life planning grief counseling and emotional support
- Coordinate community resources and referrals as needed
- Collaborate with the interdisciplinary team to ensure high-quality patient care
- Document visits and interventions in accordance with regulatory standards
Requirements
- Masters degree in Social Work (MSW) from an accredited program
- Current Ohio state license (LSW LISW or LCSW)
- Experience in hospice or palliative care preferred
- Strong knowledge of end-of-life care and community resources
- Compassionate and empathetic approach to care
- Excellent communication and organizational skills
