Your Role
As a POM Interviewer youll connect with individuals with disabilities to learn their goals strengths and needs on their terms. Youll conduct respectful private interviews review support systems and create clear summaries with actionable recommendations. Your insights help shape services that truly reflect what people want in their lives. If you value meaningful conversations and person-centered change this role is for you.
Responsibilities
Communicates well with consumers using their preferred form of communication when determining their likes interests strengths wants and needs.
Treats consumers with dignity and respect during the interviewing process and provides a comfortable and private environment to meet with consumers.
Uses skills developed during POM training to gather information about a consumers desired outcomes by reviewing documents interviewing consumers and interviewing those people that are closest to the consumer.
Assesses services and supports to identify if they are meeting the needs of the consumer to obtain desired outcomes or if there are opportunities to support meeting outcomes.
Creates a summary for each consumers POM interview that identifies what was learned for each indicator and gives an overview about the desired outcomes of the consumer.
Develops recommendations for opportunities that can support the outcomes identified by the consumer and interviewer.
Completes interviews and all needed paperwork in a timely manner.
Follows all agency Policies and Procedures.
Qualifications
Good verbal and written communication skills
Ability to effectively work with coworkers and consumers
Knowledge of working with individuals with disabilities
Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines and standard accepted practices
High School Diploma preferred
Must maintain a valid drivers license and meet agency driving requirements when required to provide transportation.
