Job Description

Job Title: Team Supervisor - Home Care
Department: Rochester Regional Home Care
Location: 330 Monroe Ave Rochester NY
Hours Per Week: 40
Schedule: Days; Monday - Friday 830 am to 5 pm

SUMMARY:
Supervises interdisciplinary staff under the general direction of the Clinical Business Leader. Responsible for the day to day clinical support to field staff as well as ensuring that patients receive timely quality care. Coordinates patient care along with office support staff; participates on Leadership Team.

RESPONSIBILITIES:

  • Supervises all professional home visiting staff assigned to the team
  • Supervises the training and development of team members. Provides clinical and professional/interpersonal training to aid in developing treatment and other skills/qualities required in practice.
  • In collaboration with the Clinical Business Leader and field PHV coordinates patients plans of care while on home care services.
  • Reviews and implements department policies and procedures as requested
  • Identifies learning needs and participates in Performance Improvement plans as requested. May provide immediate performance counseling when required.
  • Participates in program development
  • In collaboration with Clinical Business Leader monitors departmental productivity. Oversees staff and patient scheduling.
  • Implements appropriate changes to maximize department potential and insure fiscal viability.
  • Monitors assignment of patients to team members and rearranges as needed.
  • In collaboration with Clinical Business Leader develops and implements efficiency measures/systems. Works with Leadership Team to develop annual department goals and objectives consistent with RRHS goals and financial conditions.
  • Authorizes orders for replacement of staff supplies and equipment.
  • Initiates and conducts quality improvement audits to ensure standards of care are met. Enforces written policies and standards of practice regarding safe and quality patient care.
  • Monitors compliance by staff for completing mandatory training provided by RRH including but not limited to mandatories specific to home care.
  • Able to perform clinical visits to provide patient care or supervisory joint visits as indicated.

REQUIRED QUALIFICATIONS:

  • Bachelors in Physical Therapy or Bachelors in Occupational Therapy with 3 years progressively responsible therapy work experience Or Masters in Speech/Language Pathology.
  • NYS Licensure in one of the following: Physical Therapy Occupational Therapy or Speech Pathology.
  • Valid NYS Drivers license.

PREFERRED QUALIFICATIONS:

  • Physical Therapy-MS or DPT preferred.
  • Previous supervisory experience preferred.
  • Excellent communication and interpersonal skills

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking standing or squatting.

$80000.00 - $106000.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
