Sub-Acute Social Worker Job Summary

The Sub-Acute Social Worker in a Skilled Nursing Facility plays a pivotal role in ensuring the emotional psychological and social well-being of patients and their families. This position focuses on providing comprehensive social work services to subacute patients transitioning from acute care to rehabilitation or long-term care settings and subsequent discharge coordination as necessary. Their primary duties include assessing patients helping to develop appropriate treatment plans and administering therapy directly to patients. These tasks are performed with little oversight and of necessity required reliance on their independent judgment. They are a learned professional and their primary duties requires knowledge of social work acquired by a prolonged course of specialized intellectual instruction as a graduate of a school of social work accredited by the Council on Social Work Education and is certified in the State of New York and has a LMSW or LCSW.

  • Conduct comprehensive psychosocial assessments at admission to identify social emotional and environmental factors affecting care
  • Evaluate mental health conditions develop and implement individualized care plans with the interdisciplinary team
  • Update psychosocial assessments and care plans regularly (e.g. every 90 days or as needed).
  • Assess discharge potential document discharge plans and ensure they are reflected in the care plan.
  • Provide direct clinical services: individual group family and crisis counseling.
  • Facilitate adjustment to facility life and promote resident acceptance of limitations while encouraging engagement with strengths.
  • Participate in quality improvement and pre-admission evaluations to identify psychosocial risks.
  • Coordinate discharge plans with external providers and agencies.
  • Maintain knowledge of local community resources and refer residents/families as needed.
  • Stay informed about Medicare Medicaid and other financial/social programs.
  • Lead interventions to restore self-care capacity enhance mental/social functioning and promote independence.
  • Serve as liaison and advocate for residents and families regarding care facility policies rights and services
  • Support residents and families through transition discharge and understanding care plans and limitations.
  • Visit and interview newly admitted residents to aid emotional adjustment and compile social histories.
  • Collaborate with nurses physicians therapists case managers and consultants to ensure coordinated integrated care.
  • Advise staff on residents psychosocial needs and behavioral health concerns.
  • Maintain clinical documentation in accordance with regulatory standards HIPAA and facility policy
  • Record all social service interventions and psychosocial data in the medical record.
  • Attend in-services meetings seminars and trainings to stay current in gerontology long-term care and mental health.
  • Participate in facility in-service education programs.
  • Perform other related duties as required and assigned.
Requirements
  • Masters degree in Social Work (MSW) from an accredited school of social work.
  • Current licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State required.
  • Minimum of 1-2 years of social work experience in a healthcare setting preferably in a skilled nursing facility or subacute care environment.
  • Strong knowledge of psychosocial assessment counseling techniques care coordination and discharge planning
  • Demonstrated expertise in psychosocial assessment discharge planning and therapeutic intervention.
  • Ability to independently manage complex caseloads and resolve issues using sound clinical judgment.
  • Excellent communication interpersonal and organizational skills.
  • Ability to work effectively as part of an interdisciplinary team and independently manage multiple priorities.
  • Proficiency in electronic health records (EHR) and Microsoft Office Suite.
  • Pleasant personality and ability to work with residents families and staff from various departments.
  • Proficient in Microsoft Office.
  • Ability to work well under pressure and be flexible.
  • Exemplary customer service skills.
