As a Transition to Community Peer Support Specialist Team Leadyou willprovide support and supervision to the Peer Support staff and provide therapy to their consumers. The purpose of the Peer Support Specialist Transition to Community Living service is to enhance the consumers ability to function and achieve their goals while residing in an independent living/community setting. The ratio of supervisor to CPSS is 1 supervisor to 8 Peers. The Peers will have a maximum caseload of 15 clients. The individuals being served are diagnosed with a Mental Health and/or Substance Use disorder. In addition the individuals will be living in Permanent Supportive Housing and participating in the Transition to Community Living program. The intent of the service is to increase an individuals ability to live successfully in the community by maintaining tenancy.

Compensation:$57807 - $67135
*Final compensation is determined by factors such as education licensure level experience and other qualifications
